Below is a list of the most common forms used when conducting Aids to Navigation and Chart Updating activity.  Each form is listed in a PDF or MS Excel format with a small description of what the form is for.   For a complete list of all U.S. Coast Guard forms please go here.       FORM No.  FORM TITLE Revision Date Form Type Size   NS-AN04 Aid to Navigation Report 2/17/09 Excel ? Use: Use this format for reporting discrepancies observed on Federal Short Range Aids to Navigation to your CG ANT. This form will be available for download from the National Forms Web Site.  An Excel version is available on this site temporarily until the Navigation Systems Division website is updated.   FORM No.  FORM TITLE Revision Date Form Type Size   NS-AN04a Aid to Navigation Report -Field Explanations 2/17/09 ? Use: Field Explanations - Information on each form field to help you to complete NS-AN04 form.  It is used to report discrepancies observed on aids to navigation, such as buoys, lights, day beacons, ranges, and electronic aids, which are owned or operated by Federal Government.   FORM No.  FORM TITLE Revision Date Form Type Size   NS-AN10 Aton Observation Worksheet 2/17/09 Excel & ? Use: Use this worksheet while on-scene collecting data about an aid. Data fields are provided for all of the necessary pre-underway and on-scene reviews. Simply check off any observed discrepancies. This worksheet is referenced to the revised "NS-AN04 -ANSC 7054 Aid to Navigation Discrepancy Report." This worksheet is available on page 45 of the "NS-AN02 - Federal Short Range Aid to Navigation Study Guide".   FORM No.  FORM TITLE Revision Date Form Type Size   NS-BAP03 Bridge Discrepancy Report 2/20/09 Excel ? Use: This form should be used by all members to report discrepancies observed on bridges to D11 (dpw) Bridge Section via the ADSO-NS/B which are not done under the Eleventh District Annual Bridge Survey.  Eleventh District Annual Bridge Surveys by Aid Verifiers use a different form.   FORM No.  FORM TITLE Revision Date Form Type Size   NS-CU03 Chart Updating Form 2/18/09 Excel ? Use: This form is directed at Chart Updating and Coast Pilot correcting. The (locating or Fixing) LAT/LON fields and depth fields have been expanded to include the necessary quality control reporting requirements associated with these operations. Space is provided for a task statement, comments and observations, and a final recommendation to NOAA based on the evidence reported. Also, there is a place on this form for ordering a NOAA Nautical Chart.  However, be sure to note the rules associated with ordering and receiving free charts.  This form is temporarily available in Excel until it is programmed in Adobe and added to the National Forms website.               FORM No.  FORM TITLE Revision Date Form Type Size   NS-CU03A Chart Updating Form Field Explanations 2/18/09 ? Data fields are provided for the essential data for each planned observation. Prepare this worksheet on-line to enhance the professionalism of your final CU report. Explanation of each field can be found in the NS-CU02 Chart Updating Training Guide. This worksheet is temporarily available in Excel until it is programmed in Adobe and added to the National Forms website.               FORM No.  FORM TITLE Revision Date Form Type Size   NS-SC03 Small Craft Facility Worksheet (7031-B) 2/08/09 Excel & ? Use: This form was designed solely for Small Craft Facility reporting. Space is provided for recording every detail of the SCF interviewing process. Also, there is a place on this form to order a NOAA Nautical Chart. The SCF Worksheet is the document that is forwarded to NOAA (through the DSO-NS) with all the required attachments.               FORM No.  FORM TITLE Revision Date Form Type Size   NS-SC04 Small Craft Facility Worksheet Form Example (7037-B) 2/08/09 Excel & ? Use: This form is an example, it was designed solely for Small Craft Facility reporting. Space is provided for recording every detail of the SCF interviewing process. Also, there is a place on this form to order a NOAA Nautical Chart. The SCF Worksheet is the document that is forwarded to NOAA (through the DSO-NS) with all the required attachments.               FORM No.  FORM TITLE Revision Date Form Type Size   NS-CU05 Coast Pilot Text Correction Worksheet 2/18/09 Excel ? Use: Use this worksheet to organize updates and changes to the United States Coast Pilot. This worksheet helps pinpoint the page and paragraph of your changes for NOAA. The existing and new Coast Pilot copy are included as part of the report presentation. This worksheet should always be computer generated to enhance the professionalism of your final CU Update Report and is always sent to NOAA as an attachment to the NS-CU03-Chart Update Form.
